Part-Time, Virtual
Effectively support individuals with SCD across the lifespan by focusing on solutions to
acute and chronic psychosocial barriers to care. Reports to the Program Director of
Crescent Foundation. Bilingual candidates are encouraged to apply (especially those
who speak Creole, Spanish, other high need languages in the city of Philadelphia)
Responsibilities:
- Provide necessary service to Crescent Foundation clients
- Engage one-on-one with high-risk patients to set and achieve health goals
- Identify and set health goals, and follow-up provision of social support, health
system navigation, community resource linkage and behavior change counseling
- Remain autonomous and possess consistent drive for excellence, ability to make
appropriate and timely decisions with minimal supervision, and effective
problem-solving skills
- Maintain thorough and accurate documentation
- Work collaboratively within a multidisciplinary patient care team
- Participate in initiatives to advance practices in CHW.
Qualifications:
- B.S. or B.A. in any social science
- Case management experience
- Knowlege of state and federal applications: SSI, UEC benefits, SNAP, etc.
- Commitment to providing high-quality care and supportive services to individuals
living with SCD and to families across the lifespan who are underserved
Salary: $35,880
